Daniel L. Arnold, M.D., F.A.C.S., is a board-certified general surgeon. He graduated from the University of Kentucky College of Medicine and completed his residency at the University of South Florida. He is a fellow of the American College of Surgeons and the Southeastern Surgical Congress. Dr. Arnold also is a member of the American College of Phlebology and International Association for Physicians in Aesthetic Medicine.

Hugo V. Hart, M.D., F.A.C.S., is a board-certified surgeon with more than 25 years of experience. He earned his Bachelor of Arts from Southern Connecticut State University and his medical degree from the Universidad Nacional Pedro Henriquez Urena in the Dominican Republic. He completed his general surgery residency at Bridgeport Hospital. He is certified with the American College of Surgeons and The American Board of Surgery, and he is a member of the American College of Phlebology. He is currently on staff at Florida Hospital and Orlando Regional Medical Center.

Samuel P. Martin, M.D., F.A.C.S. |
Samuel P. Martin, M.D., F.A.C.S., is a board-certified vascular specialist with more than 30 years of experience. He is also a board-certified phlebologist (vein doctor). His educational background includes a bachelor’s degree from Yale University and Doctor of Medicine from Duke University. He served his general-surgery residency at New York Hospital, Cornell, and a vascular fellowship at the Ochsner Foundation Hospital. Dr. Martin’s expertise is in the nonsurgical treatment of all vein problems, including ultrasound-guided sclerotherapy, laser therapy and Veinlite sclerotherapy. These procedures produce no scarring and no downtime, and are virtually pain-free. Dr. Martin is a fellow of the American College of Surgeons and member of the American College of Phlebology. He practices at Vascular Vein Centers and is affiliated with Florida Hospital and Orlando Health.

Carlos H. Colombo, M.D. |
Carlos H. Colombo, M.D., was born and raised in South America. With his multicultural background, he is fluent in Spanish, Italian, Portuguese and English. He attended the National University of La Plata in Buenos Aires, where he received his training in emergency medicine, pathology and internal medicine. He continued his education in the U.S., with training at Framingham Union Hospital, the University of Massachusetts and Orlando Regional Medical Center. Dr. Colombo’s health care emphasis is on wellness and preventive medicine. He specializes in injuries and disability and is dedicated to the nonsurgical treatment of soft tissue trauma, mainly focusing on physical therapy and rehabilitation. Dr. Colombo also has extensive experience in court testimony and clinical quality improvement. He is active in community health education and women’s wellness. Dr. Colombo is a member of the American Medical Association.

Maryann Daniels, P.T. |
Maryann Daniels, P.T., earned her bachelor’s degree in physical therapy from the University of Central Florida in 1996. After six years of experience with neurological and orthopedic patients at Florida Hospital, she underwent advanced training in manual therapy in California for four years. While there, she worked at a private practice outpatient clinic, where she became proficient in spinal, pelvic girdle, costal and various other orthopedic dysfunctions. Daniels has been a physical therapist for 13 years. She currently practices orthopedic manual therapy at Florida Center for Orthopaedics.

Cheryl Ferguson, O.T., is an occupational therapist at Select Physical Therapy. She earned her bachelor’s at Acadia University in Canada in 1991 and an advanced master’s at Springfield College in Massachusetts in 1994. She began her career treating orthopedic upper extremity conditions and injuries at the University of Connecticut Sports Center, before exploring treatment of pediatric upper extremity functional limitations. She spent several years working in the school system and outpatient clinics treating a variety of diagnoses within the pediatric and adult population. Azeeam Shan Khaleel, P.T., was born in Montreal and raised in Brooklyn, N.Y. He moved to Florida in 1990 and attended the University of Central Florida in 1996, where he received an associate degree in health administration and fulfilled the prerequisites needed to become a physical therapist. He graduated from Fontys University of Applied Sciences in The Netherlands in 2000, where he received a bachelor’s in physical therapy. Khaleel has experience in inpatient and outpatient clinical settings treating a variety of diagnoses, including spinal cord injuries, post-surgical procedures, sports injuries and whiplash. He has received postgraduate training in cervical/lumbar spine disorders, work hardening, functional testing and job-site analysis. He is a member of the American Physical Therapy Association.
